2025 Nevada Primary Care Association Annual Health Care Awards
The Nevada Primary Care Association is proud to recognize the outstanding contributions of health center staff, board members, and community supporters who have dedicated their time, effort, and passion to advancing the Community Health Center Movement in Nevada. The Nevada PCA Annual Health Care Awards program starts with you - nominate and celebrate those who make a difference! Winners will be announced at this year's NVPCA Annual Health Care Conference in September. Any Health Center staff can nominate colleagues for the following five awards:

Exceptional Board Leadership Award
This award is presented to an individual Community Health Center board member who has demonstrated commitment and dedicated service to the Community Health Center.
Eligibility: An individual currently serving on a Community Health Center board, nominated by a Community Health Center CEO or C-suite staff member.

Exceptional Administrative Leadership Award
This award is presented to an exceptional administrator or staff member who has demonstrated commitment to the Community Health Center Movement through quality leadership and staff supervision within their Health Center.
Eligibility: An individual in an administrative leadership role at a Community Health Center.

Outstanding Provider Award
This award recognizes a distinguished provider who exemplifies excellence in clinical practice and demonstrates a dedication to the Health Center's mission and to the patients he/she serves.
Eligibility: An individual who is a medical, dental, or behavioral health provider at a Community Health Center. Nominees may include, but are not limited to, physicians, dentists, nurse practitioners, physician assistants, dental hygienists, registered nurses, nurse midwives, social workers, and pharmacists.

Unsung Hero Award
This award recognizes an individual who has demonstrated a level of excellence in his/her work that distinguishes his/her services and contributions during the past year.
Eligibility: An individual who is a staff member or volunteer at a Community Health Center who is not typically recognized publicly for his/her contributions. Nominees may include, but are not limited to, front-line staff, outreach workers, financial assistance staff, interpreters, accounting staff, and human resources staff. Nominees demonstrate a commitment to the mission of the Health Center and a dedication to providing outstanding service to Health Center patients.

Exceptional Health Professions Educator Award
The Exceptional Health Professions Educator Award recognizes and honors an exceptional individual who has consistently demonstrated exemplary teaching, mentorship, and leadership in shaping the next generation of professionals. This award celebrates dedication to fostering academic and clinical excellence, empowering students and trainees with the knowledge, skills, and ethical principles needed to succeed in their chosen field.
Eligibility: An individual who teaches, mentors, and helps support health profession trainees.

Award nominations are now open and will close on Friday, June 20, 2025, at 5:00pm PST.
Nomination Review Rubric

Nevada Primary Care Association uses a nomination review rubric to determine the Annual Health Care Awards. Nominations are rated based on these criteria:

Relevance of the nomination to the category

Number of unique nomination stories received for that individual

Commitment and dedication to the Health Center and the community
-
Commitment and impact on patients
-
Compelling story explaining why the nominee deserves this award.

Submissions must be at least one paragraph long. The story behind the winning nomination will be shared publicly and may be edited for clarity or brevity.
