WHAT IS A COMMUNITY HEALTH CENTER?
Community Health Centers (CHCs) are non-profit, community-based, and patient-directed organizations that deliver comprehensive, culturally competent, high-quality primary health care services.
CHCs also often integrate access to pharmacy, mental health, substance use disorder, and oral health services in areas where economic, geographic, or cultural barriers limit access to affordable health care services.
Anybody can receive care at a CHC. ​
Health Center Program Fundamentals:

Deliver high quality, culturally competent, comprehensive primary care, as well as supportive services such as health education, translation, and transportation that promote access to health care.

Provide services regardless of patients’ ability to pay and charge for services on a sliding fee scale.
​

Operate under the direction of patient-majority governing boards of autonomous community-based organizations. These include public and private non-profit organizations and tribal and faith-based organizations.

Develop systems of patient-centered and integrated care that respond to the unique needs of diverse medically underserved areas and populations.

Meet requirements regarding administrative, clinical, and financial operations

Provide highly efficient and cost-effective care, generating $24 billion in savings for the health care system annually.

Increase access to timely primary care, playing a role in reducing costly, avoidable emergency department (ED) visits and hospital stays. The average cost for a health center medical visit was less than one-sixth the average cost of an ED visit in 2012.

Deliver a broad array of primary and preventive care services, including screening, diagnosis and management of chronic illnesses such as diabetes, asthma, heart and lung disease, depression, cancer and HIV/AIDS.

Reduce mortality, health disparities and risk of low birth weight with the care they deliver.

Offer numerous enabling services such as transportation, translation, case management and health education in order to ensure their patients are receiving the care they need.

However, due to financial difficulties, a lot of people do not get access to high-quality medical attention. Their mission is to bridge that gap through their full-service health facilities

They provide free and low-cost outpatient medical treatments for families and individuals who have not received proper care for their conditions. The use of their clinic is free, and they offer a wide range of services such as primary care, pediatric care, and women’s health.

Canyonlands Healthcare is a private, not-for-profit corporation providing quality primary and preventative care. They provide comprehensive and cost-effective healthcare services to 10 locations in 8 communities in Arizona and Nevada.

Community Health Alliance provides services in Washoe County, Nevada. They see patients in seven health centers, two mobile units, and affiliated programs. Community Health Alliance gives people access to integrated services that promote wellness while working with you to achieve a healthy life.

FIRSTMED HEALTH & WELLNESS CENTER
FirstMed Health and Wellness Center is a non-profit organization providing preventive and primary health care services to patients in Las Vegas, Nevada. FirstMed was conceived in 2009 by a group of physicians and professionals primarily to be able to serve the uninsured and under-insured population. It is the core belief of FirstMed that a vulnerable population can be empowered through increased access to comprehensive healthcare services that help people become and stay employed, have adequate resources for healthy lives, and live in a healthy home and environment.

FIRST PERSON CARE CLINIC
The Mission of First Person Care Clinic is to provide efficient, quality, and affordable healthcare by maintaining the patient's continuity of care through a holistic, compassionate, and accessible approach to all, including medically underserved, uninsured, and underinsured individuals.

HOPE CHRISTIAN HEALTH CENTER
Hope Christian Health Center seeks to share Christ’s love among our neighbors in Vegas by providing excellent and affordable healthcare to all. Fueled by a desire to integrate our commitment to share the love of Christ in Las Vegas and to offer high-quality health care to these same neighbors, a group of medical professionals and others from local churches have established a Christian Health Center at the corner of Pecos & Craig in North Las Vegas.

NORTHERN NEVADA HOPES
Northern Nevada HOPES is a nonprofit community health center in downtown Reno, NV that offers integrated medical care and wellness services. They welcome patients, wherever they are in life, and provide them with compassionate, high-quality healthcare. Their growing list of services includes primary care, chronic disease management, behavioral health counseling, social services, an on-site pharmacy, and more. HOPES' team-based approach to care gives patients access to a range of comprehensive services in one central location. For medically underserved populations, their one-stop shop healthcare model reduces barriers to care and increases the likelihood of maintaining long-term health. HOPES accepts most insurance plans, Medicaid, Medicare, and the uninsured. For the uninsured, they offer discounted services based on a sliding fee scale.

NEVADA HEALTH CENTERS
Nevada Health Centers is a nonprofit organization that provides healthcare services to more than 50,000 babies, children, and adults in city, rural, and frontier communities throughout Nevada each year. Established in 1977, Nevada Health Centers is proud to be one of the first federally funded Community Health Centers, as authorized under the Public Health Services Act of 1976. In keeping with their mission of providing access to quality health care services throughout Nevada, Nevada Health Centers is the largest provider of primary care for the uninsured, underinsured, and/or geographically isolated people in the state - in some cases, their rural and frontier clinics are the only places for people to get healthcare without having to travel hundreds of miles. Nevada Health Centers is able to offer a sliding fee scale based on federal poverty levels.

NORTH EAST MEDICAL SERVICES
North East Medical Services (NEMS) is a non-profit community health center serving the San Francisco Bay Area. With locations throughout the San Francisco Bay Area as well as in Las Vegas, their mission is to provide affordable, comprehensive, compassionate and quality health care services in a linguistically competent and culturally sensitive manner to improve the health and well-being of our community.
SAFE HARBOR MEDICAL
Safe Habor Medical is the trusted non-profit community health center in Nevada focused on the needs of their community, providing comprehensive and compassionate healthcare services spanning all life stages—from pediatrics to geriatrics. It began as Harbor Island Medical Center @ Summerlin, established by a provider group in 2007. In 2021, the providers wanted to retire and leave a legacy for their patients. The company was sold to Tina Alicea, who converted the beloved practice into a non-profit community health center; moved the clinic to a larger medical facility closer to ancillary services; grew the provider staff, and expanded the services while building a patient and community-focused board of directors, and received FQHC designation August 2024. The result is Safe Harbor, whose mission is to provide exceptional healthcare services to their community from preventative care to patient primary care and specialized treatments.

SOUTHERN NEVADA COMMUNITY HEALTH CENTER
Southern Nevada Community Health Center is located within Southern Nevada Health District. The mission of the health district is, “To assess, protect, and promote the health, the environment, and the well-being of Southern Nevada communities, residents, and visitors.” To this end, the health district offers services and provides regulatory supervision that impacts the public every day.
