1. Make An Appointment
Your first visit to a community health center begins with making an appointment to visit a health specialist. Most health centers offer walk-in appointments, but can also schedule appointments for a specific time and date.
2. After your Appointment is Scheduled
Please arrive 15 to 30 minutes prior to your appointment. You will need time to fill out the initial paperwork.
3. What Items you Should Bring
Driverâ€™s License or Photo I.D.
List of current medications including vitamins and nutritional supplements.
Proof of income *
* Proof of income is required for patients wishing to utilize the Sliding Fee Scale. The Sliding Fee Scale provides discounts for primary care based on income and other factors.
